Nagging Shoulder Blade Pain? It Might Be Coming from Your Neck
If you’ve been dealing with persistent pain around your shoulder blade that just won’t go away, you might be surprised to learn that the problem isn’t actually in your shoulder. In many cases, nagging pain in this area is actually referred pain from the cervical spine—specifically the cervical facet joints.
Understanding Cervical Facet Referral Pain
The cervical facet joints are small joints in the neck that allow for movement and stability. When these joints become irritated due to poor posture, muscle imbalances, or repetitive strain, they can refer pain down into the shoulder blade, upper back, and even the arm.
Each cervical facet joint has a specific referral pattern, meaning that dysfunction in different areas of the neck can cause pain in different parts of the upper body. For example:
C4-C5 facet dysfunction can send pain to the upper shoulder blade.
C5-C6 dysfunction can refer pain down the mid-scapular region.
C6-C7 irritation can send pain lower along the inner border of the scapula or even into the arm.
Why Does This Happen?
The nerves that supply sensation to the neck and upper back are interconnected. When a cervical facet joint becomes inflamed or irritated, the nervous system can misinterpret the pain signal, making it feel like the discomfort is coming from the shoulder blade rather than the actual source—the neck.
Common Causes of Cervical Facet Referral Pain
Poor Posture – Forward head posture, slouched shoulders, and excessive screen time can put strain on the cervical spine.
Muscle Imbalances – Weak deep neck flexors and tight upper trapezius and levator scapulae muscles can create excessive stress on the facet joints.
Repetitive Movements – Prolonged desk work, frequent overhead activities, or carrying heavy loads can aggravate the cervical spine.
How to Address the Root Cause
Instead of chasing the pain in your shoulder blade, addressing the cervical spine by unloading it can provide lasting relief. Here’s how:
✅ Postural Correction – Improving head and shoulder alignment reduces strain on the neck and facet joints.
✅ Core & Scapular Stability Exercises – Strengthening your core and scapular stabilizers helps offload stress from the cervical spine.
✅ Manual Therapy & Joint Mobilization – Techniques such as soft tissue work, joint mobilization, and dry needling can help release tension and improve mobility.
✅ Ergonomic Adjustments – Optimizing workstation setup and movement patterns reduces daily strain on the cervical spine.
